Your A-PLAN comes with personalised coaching to ensure you're supported every step of the way:

Duration: 6 months

Format:

  • An initial one-hour consultation that includes MoCA cognitive and Healthy Lifestyle Assessments to establish risk factors
  • Follow-up with your assessment results and develop you’re a-PLAN
  • 3 further meetings at months 2,4,& 6 to assess A-PLAN impact
  • Re-assess your Alzheimer’s Risk Profile after 6 months

This will show what improvements have taken place and how we need to proceed.
